One must first understand the correct meaning of the original. It's not an interchange (diamond or otherwise) but a road fork. Interestingly enough, in English the metaphor refers to the roadway lanes (fork tines), while in Portuguese (needle) it refers to the dividing element, in this case a traffic island at the Rebouças ramp. Granted, the Portuguese original is not a model of precision and proper grammar but it's serviceable. In translation:

Continue on Av. Epitácio Pessoa, in the direction of (Túnel) Rebouças, but do not take the tunnel access ramp. Rather, bear right on the fork and continue until the stoplight.
