Verrà usata una terminologia medica standard per registrare tutte le affezioni attuali sotto forma di diagnosi assieme alle affezioni, agli interventi ed alle procedure diagnostiche o terapeutiche di rilievo subite in passato. Verrà anche registrata la anamnesi cardiovascolare che comprenderà la documentazione della classe NYHA (si veda la Appendice D). La anamnesi recente verrà raccolta in occasione di ciascuna visita successiva e qualsiasi informazione di rilievo (per es. eventi avversi) verrà registrata nella Sezione appropriata della Scheda Raccolta Dati. La valutazione della classe NYHA verrà effettuata alla fine di ciascuna Fase di Incremento della Dose, al 6°, 12° e 18° mese della Fase di Mantenimento ed in occasione di un eventuale ritiro prematuro dallo studio.
Translation - English All pertinent information regarding medical history will be reported at baseline/initial selection. Standard medical terminology must be used for reporting all current diseases in diagnostic form along with any diseases, operations, notable diagnostic or therapeutic procedures undergone in the past. The cardiovascular history of the patient will also be reported along with documentation of the NYHA class (see Appendix D). The recent medical history will be gathered during each subsequent visit and all important information (e.g. adverse events) will be reported in the appropriate Section of the Case Report Form. Assessment of the NYHA class will be conducted at the end of each Dose Escalation Phase, at the 6th, 12th and 18th month of the Maintenance Phase as well as in the event of any early withdrawal from the study.

In 2008 I completed a master's course in medical & pharmacological translation in Italy for translations of this type: clinical trials, informed consent, package leaflets, medical reports, publications, veterenary sciences. I have a degree in Psychology from UCLA as well as a minor in French and studied in France for 1 year. I lived in Italy for 10 years and have done IT>EN translations for about 8 years. I am also married to an Italian, and we always speak in Italian so that I can keep my skills fresh. I also speak excellent Spanish since I lived in a Spanish speaking country for a few years as a teenager.
