on a baptism certificate, a father's name is provided, immediately followed by "ban." and then there is space, so "ban." is not filled out.

Means "who lived" (bixit) "years" (annos). SOmetimes also b.an.
Obviously those who filled in the form didn't know the birth (or death) year of the father.
