OBJETIVO
Brindar los lineamientos para las visitas de seguimiento a los programas de Farmacovigilancia con que deben contar los establecimientos pertenecientes a la Red Nacional de Farmacovigilancia, de acuerdo a lo regulado en la Resolución 2004009455 de 28 de mayo de 2004 (Titulares de Registro Sanitario, Fabricantes entre otros establecimientos pertenecientes a la industria farmacéutica) y del Decreto 2200 de 2005, Resolución 1403 de 14 de mayo de 2007 y Resolución 2003 de 2014 (Instituciones Prestadoras de Servicios en Salud y demás definidas en esta normatividad) y definir los criterios de evaluación que se aplicarán para la ejecución de dicho seguimiento.

ALCANCE
La presente guía es aplicable para comunicar el desarrollo de las visitas de seguimiento al Programa de Farmacovigilancia realizadas a Titulares de Registro Sanitario, Fabricantes y demás establecimientos estipulados en la Resolución No. 2004009455 de 28 de mayo de 2004 y también para las que se realizan a las Instituciones Prestadoras de Servicios de Salud (IPS) y demás establecimientos definidos en el Decreto 2200 de 2005, Resolución 1403 de 2007 y Resolución 2003 de 2014.
3.
DEFINICIONES
Acción correctiva:
Acción para eliminar la causa de una no conformidad y evitar que vuelva a ocurrir.1
Alerta sanitaria:
Sospecha de una situación de riesgo potencial asociada a la utilización de un medicamento y/o Producto Biológico, que pueda afectar la salud de la población o de trascendencia social, frente a la cual se hace necesario el desarrollo de acciones de Salud Pública urgentes y eficaces.2
Evento adverso:
Cualquier suceso médico desafortunado que puede presentarse durante un tratamiento con un medicamento, pero que no tiene necesariamente una relación causal con el mismo.3
Evento adverso inesperado:
Aquel suceso del cual no se tiene un conocimiento previo hasta la fecha de su ocurrencia por no haber sido consignado en la información de seguridad de un medicamento.4
Evento adverso serio:
Se considera serio a un evento adverso cuando su desenlace es la muerte, una amenaza a la vida, cuando genera o prolonga una hospitalización, causa discapacidad, interrupción del embarazo o genera malformaciones.4
Establecimiento farmacéutico:
Es el establecimiento dedicado a la producción, almacenamiento, distribución, comercialización, dispensación, control o aseguramiento de la calidad de los medicamentos, dispositivos médicos o de las materias primas necesarias para su elaboración y demás productos autorizados por ley para su comercialización en dicho establecimiento.3
Fabricación:
Todas las operaciones que incluyan la adquisición de materiales y productos, producción, control de la calidad, autorización de circulación, almacenamiento, embarque de productos acabados, y los controles relacionados con estas operaciones.5

Ciencia y actividades relacionadas con la detección, evaluación, entendimiento y prevención de los eventos adversos o cualquier otro problema relacionado con medicamentos.3
Informe periódico de beneficios y evaluación de riesgos - PBRER:
Documento descrito en la guía ICH E2CR2, que pretende ser una norma común para los informes periódicos de evaluación de riesgo-beneficio sobre los productos comercializados (incluidos los medicamentos aprobados que se encuentran bajo estudio adicional), entre las regiones de la ICH.
Las agencias reguladoras de la Unión Europea, Japón y los EE.UU. consideran que el PBRER puede ser utilizado para satisfacer las necesidades nacionales y regionales imperantes en materia de seguridad periódica y/o informes de riesgo-beneficio de los medicamentos aprobados.6
Informe Periódico de Seguridad - PSUR:
Es un documento que tiene como objetivo el proporcionar en forma actualizada la experiencia global de la seguridad un medicamento específicos después de su aprobación.
Estos informes fueron propuestos por el “International Council of Harmonisation (ICH)” y consolidado en las guías conocidas como ICH E2C con la finalidad de:
1) Reportar toda nueva información de seguridad de fuentes apropiadas.
2) Relacionar estos datos con la exposición del paciente.
3) Resumir el estado de la autorización en diferentes países y cualquier variación relacionada con la seguridad.
4) Crear periódicamente la oportunidad de una reevaluación global de la seguridad.
5) Indicar si se deben hacer cambios a la información del producto con el fin de optimizar su uso.7
Institución Prestadora de Salud - IPS:
Son entidades oficiales, mixtas, privadas, comunitarias y solidarias, organizadas para la prestación de los servicios de salud a los afiliados del Sistema General de Seguridad Social en Salud, dentro de las Entidades Promotoras de Salud o fuera de ellas.
Son entidades organizadas para la prestación de los servicios de salud, que tienen como principios básicos la calidad y la eficiencia, cuentan con autonomía administrativa, técnica y financiera, y deben propender por la libre concurrencia de sus acciones.8
Medicamento:
Es aquel preparado farmacéutico obtenido a partir de principios activos, con o sin sustancias auxiliares, presentado bajo forma farmacéutica, que se utiliza para la prevención, alivio, diagnóstico, tratamiento, curación o rehabilitación de la enfermedad.
Los envases, rótulos, etiquetas y empaques hacen parte integral del medicamento, por cuanto éstos garantizan su calidad, estabilidad y uso adecuado.9
Medidas Sanitarias:
Conjunto de medidas de salud pública y demás precauciones sanitarias aplicadas por la autoridad sanitaria, para prevenir, mitigar, controlar o eliminar la propagación de un evento que afecte o pueda afectar la salud de la población.10
Nodos de Farmacovigilancia (NFV):
Entidades ó instituciones que hayan establecido más de un contacto vía electrónica, escrita o telefónica con el Programa Nacional del Invima para el envío de información científica, comunicaciones de interés y/o reportes de eventos adversos.
El nodo central será el Invima.
Según el grado de desarrollo de los programas se establecerán nodos regionales.

Problema Relacionado con Medicamentos (PRM):
Es cualquier suceso indeseable experimentado por el paciente que se asocia o se sospecha asociado a una terapia realizada con medicamentos y que interfiere o potencialmente puede interferir con el resultado deseado para el paciente.3
Programa Nacional de Farmacovigilancia (PNFV):
Programa que enfoca sus esfuerzos en la perspectiva epidemiológica y social de los problemas asociados al uso de los medicamentos y sus efectos, con el fin de prevenirlos y resolverlos.
Este programa está a cargo del Instituto Nacional de Vigilancia de Medicamentos y Alimentos – Invima 12.
Red Nacional de Farmacovigilancia (RNFV):
Es el conjunto de personas e instituciones que mantienen contacto entre sí a través de reportes de eventos adversos, comunicaciones e información en relación con problemas de seguridad o uso correcto de medicamentos.
La red incorpora personas o instituciones que han establecido contactos permanentes con el programa de Farmacovigilancia del Invima.13
Registro Especial de Prestadores en Servicios de Salud (REPS):
Es la base de datos de las entidades departamentales y distritales de salud, en la cual se efectúa el registro de los Prestadores de Servicios de Salud que se encuentren habilitados, la cual es consolidada por el Ministerio de Salud y Protección Social.
De conformidad con lo señalado por el artículo 56 de la Ley 715 de 2001, las Entidades Departamentales y Distritales de Salud realizarán el proceso de inscripción en el Registro Especial de Prestadores de Servicios de Salud.14
Relación Riesgo/Beneficio:
Proporción entre los beneficios y los riesgos que presenta el empleo de un medicamento.
Sirve para expresar un juicio sobre la función del medicamento en la práctica médica, basado en datos sobre su eficacia e inocuidad y en consideraciones sobre su posible uso abusivo, la gravedad y el pronóstico de la enfermedad.4
Reportante primario:
Profesional de la salud que entra en contacto directo con el paciente, identifica un Problema Relacionado con Medicamentos o Evento Adverso y lo informa al Titular de Registro Sanitario y/o al fabricante.4
Reporte:
Medio por el cual un reportante notifica sobre un evento adverso sucedido a un paciente o a un sistema de farmacovigilancia.4
Señal:
Información reportada sobre una posible relación causal entre un evento adverso y un fármaco, siendo desconocida o incompletamente documentada previamente.
Usualmente, más de un único reporte se requiere para generar una señal, dependiendo de la severidad del evento y de la calidad de la información.4

LINEAMIENTOS PARA LA PROGRAMACIÓN DE VISITAS DE SEGUIMIENTO AL PROGRAMA DE FARMACOVIGILANCIA
Las visitas de seguimiento serán realizadas y/o priorizadas a los establecimientos en los siguientes casos:
Establecimientos que no se encuentren inscritos en la Red Nacional de Farmacovigilancia y que hayan sido identificados previamente a través del REPS y del Mapa de Riesgos de la Dirección de Medicamentos y Productos Biológicos.
Establecimientos pertenecientes a la Red Nacional de Farmacovigilancia que no hayan recibido visita de seguimiento a sus programas de Farmacovigilancia en el año inmediatamente anterior.
Establecimientos que hayan sido objeto de denuncias y quejas presentadas al Invima relacionadas con aspectos que incidan primordialmente sobre la seguridad y eficacia de los productos y/o procesos que tienen a cargo.
Establecimientos titulares de Registros Sanitarios, Laboratorios farmacéuticos / Fabricantes, Importadores de medicamentos y establecimientos en general, que estén relacionados con alertas y/o medidas sanitarias y que fabriquen y/o manejen productos considerados de alto riesgo (productos biológicos, vacunas, estrecho margen terapéutico, productos nuevos de comercialización reciente en el país, entre otros.)
Nivel de complejidad:
Para el caso de las IPS, se tendrán priorizadas las instituciones de mayor nivel de complejidad por considerar que tienen un mayor impacto en la comunidad a nivel de cobertura.
Establecimientos que no cuenten con reportes o en el último año hayan presentado reportes en cero para eventos adversos a medicamentos.
Establecimientos que en su visita previa, hayan obtenido como concepto “No implementado”, de acuerdo a la evaluación hecha por los funcionarios del Grupo de Farmacovigilancia con los criterios establecidos en el numeral 6 de esta guía.
Establecimientos que se encuentren priorizados por la Unidad de Riesgos de la Dirección de Medicamentos y Productos Biológicos al haber sido calificados bajo una categoría definida como de alto riesgo, conforme a los resultados de la evaluación del Mapa de Riesgos del Invima.
Establecimientos en general que, por conocimiento de otros procesos de IVC, hayan presentado incumplimiento a lo estipulado en la normativa nacional vigente aplicable a Farmacovigilancia.

En el establecimiento visitado se expondrán los objetivos de la visita, se evaluará la totalidad o componentes en específico del Programa de Farmacovigilancia y se solicitará la documentación y soportes correspondientes, de conformidad con la normatividad que resulte aplicable para el tipo de establecimiento, según los requerimientos expuestos en los numerales 5 y 6 de la presente guía.
Concluida la visita, los funcionarios del Invima elaborarán el Acta de acuerdo al tipo de establecimiento (IVC-VIG-FM051 o IVC-VIG-FM049) con las observaciones del seguimiento realizado.

Notificación del Acta de vista
Leer y notificar los hallazgos de la visita consignados en el acta, a las personas del establecimiento que la atendieron.
Hacer entrega de la copia del acta debidamente firmada, al interesado.
En caso de que las personas que atienden la inspección, se rehúsen a firmar el acta, se dejará constancia en la misma, anexando el siguiente párrafo:
“Se deja constancia que quienes atendieron la visita se negaron a suscribir el acta.

Se generan dos copias del acta de visita, una para las personas que atendieron la visita y la otra para los funcionarios que llevaron a cabo la auditoria.

Concepto de la visita
Si el establecimiento presentó cumplimiento de los ítems definidos como MAYORES y CRÍTICOS, como concepto de la visita, se determinará que el establecimiento cuenta con un Programa de Farmacovigilancia IMPLEMENTADO.
En caso de incumplimiento de alguno de los ítems mencionados, el mismo se determinará como NO IMPLEMENTADO.

Actualización base de datos
De acuerdo al tipo de establecimiento visitado, se actualizará el formato de seguimiento a los Programas de Farmacovigilancia.
Para las visitas que hayan sido realizadas en establecimientos de la industria farmacéutica se diligenciará el FORMATO DE REGISTRO DE VISITAS - SEGUIMIENTO AL PROGRAMA DE FARMACOVIGILANCIA - INDUSTRIA FARMACEUTICA IVC-VIG-FM048 y para las instituciones de salud se diligenciará el FORMATO DE REGISTRO DE VISITAS - SEGUIMIENTO AL PROGRAMA DE FARMACOVIGILANCIA - INSTITUCIONES PERTENECIENTES A LA RED NACIONAL DE FARMACOVIGILANCIA IVC-VIG-FM050, en donde se consolidará toda la información de las visitas de seguimiento realizadas a través del año.

Acciones posteriores a la visita
En caso de que el establecimiento como resultado de la visita cuente con un concepto de NO IMPLEMENTADO luego de la revisión de la tenencia del Programa de Farmacovigilancia, éste se tendrá presente dentro de la evaluación del Mapa de Riesgos institucional para posteriores acciones de IVC y para posteriores seguimientos al mismo.
Se remitirá oficio a otras dependencias de la Dirección de Medicamentos y Productos Biológicos y demás instancias pertinentes del Invima cuando amerite o si esto es requerido por algún ente externo competente en el tema.

CRITERIOS DE EVALUACION EN VISITAS
Cada uno de los ítems con los cuales se realiza el seguimiento al programa de Farmacovigilancia cuenta con un criterio de evaluación, como se describe a continuación:
CRITICO (C):
El cumplimiento del ítem es relevante y obligatorio por parte de los establecimientos en la visita para que se determine que cuenta con un Programa de FV implementado.
Tiene un impacto alto sobre la seguridad de los productos que se manejan en el establecimiento.
MAYOR (M):
El cumplimiento del ítem debe estar debidamente soportado por parte de los establecimientos para que se determine que cuenta con un Programa de FV implementado.
Tiene un impacto medio sobre la seguridad de los productos que se manejan en el establecimiento.
MENOR (m):
El incumplimiento del ítem no es necesario para determinar que el establecimiento cuenta con un Programa de Farmacovigilancia implementado.
Tiene un impacto bajo sobre la seguridad de los productos que tiene a cargo el establecimiento.
INFORMATIVO (I):
El cumplimiento del ítem no tiene impacto sobre la definición de si cuenta o no con un Programa de FV implementado.
No obstante, es importante para contextualizar y obtener información adicional de los procesos que lleva a cabo el establecimiento auditado.

El desarrollo de estas estrategias se podrá evidenciar a través de boletines, informes, guías, folletos informativos, entre otros, relacionados con el uso seguro de medicamentos dirigidos tanto a la comunidad como a otros profesionales de la salud, con una periodicidad establecida.
Otras evidencias de este ítem podrán ser el empleo de stickers para la identificación de medicamentos (en donde se informe sobre la forma correcta de utilización para la administración segura de los mismos, identificación de medicamentos LASA y de alto riesgo, etc.), desarrollo de programas de seguridad al paciente, reuniones breves con contenidos propios de Farmacovigilancia, fondos de pantalla en computadores de la institución con temas de interés, etc.
Además, se deberá identificar como se comunica y mitiga el riesgo asociado al uso de medicamentos, a través de guías de uso seguro de medicamentos, seguimiento fármaco-terapéutico, guías de identificación de interacciones, entre otras estrategias.

ESTADISTICA DE EVENTOS ADVERSOS

Información en medio físico y/o magnético con los informes de estadísticas recientes, actas de reunión de Comités de Farmacovigilancia y/o de Comités de Farmacia y Terapéutica en donde se presentan los tipos de estadísticas que se manejan dentro de la institución, informes con acciones tomadas al respecto derivados del análisis de la estadística de los eventos adversos, detección de señales, presentaciones y discusiones con el personal que labora en la institución, entre otras.

DOCUMENTACION DEL PROGRAMA DE FARMACOVIGILANCIA

A modo general, el programa de Farmacovigilancia de la institución debe contar con un objetivo, alcance, responsables, marco legal (normatividad aplicable a Farmacovigilancia), definiciones, procedimiento estandarizado con las actividades y responsables para la detección, análisis, gestión, registro y procesamiento de eventos adversos, análisis clínico de la información, periodicidad y notificación de reportes a los entes reguladores correspondientes.
Adicionalmente, debe contar con un capítulo o ítem dedicado a la revisión, registro documental y difusión institucional de las alertas y medidas sanitarias emitidas por el Invima y por otros entes reguladores nacionales e/o internacionales.
Dentro de los anexos, se deben contemplar los flujogramas, instructivos y guías que complementen el programa y faciliten su implementación.
Ejemplo de evidencia:
Manual o documento del Programa de Farmacovigilancia en el que se describa el procedimiento de detección, análisis y notificación de eventos adversos y la implementación del formato nacional de reporte vigente del Invima o formatos propios de la institución para la realización del reporte de eventos adversos a medicamentos, análisis documentado de las estadísticas derivadas del sistema de Farmacovigilancia, último envío de reportes de eventos adversos recientemente detectados y notificados tanto al Invima como al Ente Territorial de Salud correspondiente.

REPORTES DE EVENTOS ADVERSOS A MEDICAMENTOS

Para la notificación y envío de los reportes de eventos adversos se adopta un formato propio de la institución, el formato vigente del Invima, o se realizan a través de la plataforma de reporte en línea que contiene la información básica para su adecuado análisis.

La institución debe realizar la notificación y reporte de eventos adversos a través de la plataforma de reporte en línea dispuesta por el Invima o utilizar un formato de referencia o propio con la información básica solicitada en el formato vigente de reporte nacional del Invima.
El formato de reporte de los eventos adversos de cada institución debe contar como mínimo, con la siguiente información relacionada con:
a) Paciente:
Identificación, iniciales del nombre, género y edad, diagnóstico, enfermedades concomitantes, paraclínicos y demás información que sea relevante.
b) Medicamento / Producto sospechoso y medicación concomitante:
Para cada medicamento / producto señalar indicación, fecha de inicio y final del tratamiento, dosis, vía, frecuencia de administración, momento en que se suspendió su utilización, hubo re-exposición o si continúa empleando el producto.
c) Información del evento adverso:

7.2.
Los reportes institucionales de sospechas de eventos adversos a medicamentos se remiten al Invima o quien haga sus veces y a la entidad territorial de salud correspondiente con la periodicidad establecida en la normatividad vigente para eventos adversos serios y no serios, esperados e inesperados.

Tipo de estadísticas de eventos adversos que se manejan en el establecimiento, informes de estadísticas recientes de los eventos adversos de los que se haya tenido conocimiento, actas de reunión de comités y/o presentaciones en donde se socialicen estos informes con las acciones tomadas al respecto, entre otras estrategias.
4.3.
Se realiza revisión sistemática de la literatura científica y de cualquier otra información sobre eventos adversos notificados.

El establecimiento cuenta con revisión sistemática de la literatura científica y de cualquier otra fuente bibliográfica de información de seguridad sobre los eventos adversos que le permite el análisis, la toma de decisiones y generación de planes de mejora relacionados con sus productos.

Registros documentados de revisiones en el sitio web del Invima y de otras fuentes bibliográficas reconocidas sobre información de eventos adversos, artículos sobre el perfil de seguridad de productos, alertas y medidas sanitarias, informes de seguridad, comunicados a profesionales de la salud, pacientes y cuidadores, comunicados por parte de otras agencias sanitarias de referencia, entre otros que permitan la evaluación de la causalidad y el análisis de los eventos adversos detectados por el sistema de Farmacovigilancia del establecimiento.

DOCUMENTACION DEL PROGRAMA DE FARMACOVIGILANCIA

Dicho programa deberá ser un documento diferenciado de otros procedimientos y procesos que se lleven a cabo en el establecimiento y contar con un objeto, alcance, responsables, marco legal, definiciones, procedimiento estandarizado con las actividades y los responsables para la detección, análisis, gestión, registro y procesamiento de eventos adversos, análisis clínico de la información, periodicidad y notificación de reportes a la entidad reguladora, procesamiento de alertas y medidas sanitarias, cronograma de capacitaciones en temas de Farmacovigilancia, entre otros.
De ser necesario, deberá contemplar anexos, flujogramas, guía, instructivos y formatos que complementen el programa y faciliten su implementación.
Ejemplo de la evidencia:
Manual, procedimiento o documento del Programa de Farmacovigilancia normalizado y actualizado para la notificación, registro y procesamiento de eventos adversos, alertas y medidas sanitarias, análisis de la información de seguridad de productos, envío de reportes a la entidad reguladora correspondiente y demás actividades que resulten primordiales para la adecuada implementación del mismo.
5.2.
Se elaboran Informes Periódicos de Seguridad y Planes de Gestión de Riesgo (si aplica)

REPORTES DE EVENTOS ADVERSOS A MEDICAMENTOS

7.1.
Para la notificación y envío de los reportes de eventos adversos se adopta un formato de referencia que contenga como mínimo la información básica del formato de reporte vigente estipulado por el Invima, o se realizan a través de la plataforma de reporte en línea para su adecuado análisis.

b) Medicamento / Producto sospechoso y medicación concomitante:
Para cada medicamento señalar indicación, fecha de inicio y final del tratamiento, dosis, vía, frecuencia de administración, momento en que se suspendió su utilización, si hubo re-exposición o si continúa empleando el producto.
c) Información del evento adverso:
Inicio, descripción breve, severidad y desenlace del paciente con el evento.
d) Reportante:
Nombre y domicilio del establecimiento reportante, nombre y/o identificación del notificador, profesión y contacto.
e) Análisis de causalidad del evento adverso según los criterios de UPPSALA / OMS.
Ejemplo de evidencia:
Reporte en formato de referencia (en medio físico o magnético) o a través de la plataforma en línea para los eventos adversos con la información antes descrita.
7.2.
Los reportes de sospechas de eventos adversos se remiten al Instituto Nacional de Vigilancia de Medicamentos y Alimentos – Invima o quien haga sus veces con la periodicidad establecida en normatividad vigente.

OBJECTIVE
Provide the guidelines for follow-up visits to Pharmacovigilance programs that the institutions of the Pharmacovigilance National Network should have, according to the regulations of Resolution 2004009455 of 28 May, 2004 (Health Registration Holders, Manufacturers and other institutions that are part of the pharmaceutical industry) and Decree 2200/2005, Resolution 1403 of May 14, 2007 and Resolution 2003/2014 (Institutional Healthcare Provider and others defined in this regulation) and define the evaluation criteria to be applied for conducting such follow-up.
2.
SCOPE
This handbook is designed to provide information regarding the progress of follow-up visits with regards to the Pharmacovigilance Program for health registration holders, manufacturers and other establishments listed in Resolution No. 2004009455 of the 28 of May 2004 and also for those that are carried out for Health Service Providers and other establishments listed in the Decree 2200 of 2005, Resolution 1403 of 2007 and in the Resolution 2003 of 2014.
3.
DEFINITIONS
Corrective action:
Action to eliminate the cause of a non-conformity and prevent it from happening again.1
Health Alert:
Suspicion of a potential risk situation associated with the use of a drug and/or Biological Product which may affect the population’s health or have social significance, for which implementing urgent and effective Public Health actions is necessary.2
Adverse event:
Any unfortunate medical episode that might arise during treatment with a drug, but that does not necessarily have a causal relationship with such treatment.3
Unexpected adverse event:
Any event that is unknown until its occurrence, as it is not listed in the drug safety information.4
Serious adverse event:
An adverse event is considered serious when it results in death, is life-threatening, generates or extends a hospitalization, causes disability, interruption of pregnancy or results in malformations.4
Pharmaceutical company:
A establishment dedicated to the production, storage, distribution, marketing, dispensing, control or quality assurance of the drugs, medical devices or the necessary raw materials for their production, and all the other products authorized by law for marketing in such a establishment.3
Manufacturing:
All the operations that involve the acquisition of materials and products, production, quality control, transit authorization, storage, shipment of finished products, and controls related to these operations.5

Pharmacovigilance:
Science and activities related to the detection, assessment, understanding and prevention of adverse events or any other problem associated with drugs.3
Periodic Benefit-Risk Evaluation Report - PBRER:
Document described in the guideline ICH E2CR2, which intends to be a common standard for periodic benefit-risk evaluation reports on marketed products (including drugs approved that are under additional study) in ICH regions.
Regulatory agencies in the European Union, Japan and the US consider that PBRER may be used to meet the prevailing national and regional requirements regarding periodic safety and/or risk-benefit reports of approved drugs.6
Periodic Safety Update Report - PSUR:
A document which main purpose is to provide in an updated manner the safety global experience of a drug, following its approval.
These reports were proposed by the "International Council of Harmonisation (ICH)" and consolidated in the guidelines known as ICH E2C, with the purpose of:
1) Report all the new safety information from appropriate sources.
2) Relate this data with the patient exposure.
3) Summarize the authorization status in different countries and any security-related variation.
4) Create periodically the opportunity for a safety global reassessment.
5) Indicate whether to make changes to the product information in order to optimize its use.7
Healthcare Provider Institution - IPS (by its initials in Spanish):
Official, mixed, private, public and charitable entities, organized for the provision of health services to members of the Health Social Security General System, within or outside of Health Promotion Entities.
They are entities in charge of the provision of health services, which count on quality and efficiency as basic principles, as well as administrative, technical and financial autonomy, and they must fight against anti-competitive practices.8
Medicinal product:
Pharmaceutical product obtained from active ingredients, with or without auxiliary substances, delivered in a pharmaceutical form used for prevention, relief, diagnosis, treatment, cure or rehabilitation of a disease.
Containers, labels and packages are an integral part to a drug, as they guarantee their quality, stability and proper use.9
Sanitary measures:
Set of public health measures and other sanitary precautions applied by the health authority to prevent, mitigate, control or eliminate the spread of an event that affects or may affect the population’s health.10
Pharmacovigilance nodes (NFV):
Entities or institutions that have established more than one contact by email, mail or telephone with the Invima’s National Program for sending scientific information, communications of interest and/or reports of adverse events.
The central node will be Invima.
Depending on the program development level, regional nodes will be established.
EPS [health insurance companies], IPS, drug’s health registration holders, educational institutions and groups of users and professionals will be the local, individual or institutional nodes.
Isolated individuals will be part of the network and act as nodes.11

Drug-related problems (DRPs):
Any undesirable event experienced by the patient associated or presumptively associated to a therapy with drugs, which interferes or potentially interferes with the expected result for the patient.3
National Pharmacovigilance Program (NPP):
Program that focuses its efforts on the epidemiological and social perspective of problems associated with the use of drugs and its effects, in order to prevent and resolve them.
Instituto Nacional de Vigilancia de Medicamentos y Alimentos – Invima is in charge of this program12.
National Pharmacovigilance Network (NPN):
Group of people and institutions that keep contact with each other through adverse event reports, communications and information regarding safety problems or proper use of drugs.
The network includes individuals or institutions that have established permanent contacts with the Invima's pharmacovigilance program.
Special Registry of Health Service Providers (REPS, by its initials in Spanish).:
Database of departmental and district health entities over which the registration of authorized Health Service Providers is performed, which is established by the Ministry of Health and Social Protection.
In accordance with the provisions of Article 56 of Act 715/2001, the Departmental and District Health Entities will conduct the registration process in the Special Registry of Health Service Providers.14
Risk/Benefit Relationship:
Proportion between benefits and risks of the drug use.
It serves to express a judgment on the drug’s role in the medical practice, based on efficacy and safety data, and considerations on its abusive use, severity, disease prognosis, etc.4
Primary reporter:
Healthcare professional in direct contact with the patient, who identifies a DRP or adverse event and reports it to the health registration holder and/or manufacturer.4
Report:
Means by which a reporter notifies the pharmacovigilance system about an adverse event occurred on a patient.4
Signal:
Reported information, unknown or previously documented incompletely, about a possible causal relationship between an adverse event and a drug.
Usually more than one report is required to generate a signal, depending on the severity of the event and the quality of the information.4

GUIDELINES FOR SCHEDULING FOLLOW-UP VISITS TO THE PHARMACOVIGILANCE PROGRAM
Follow-up visits will be performed and/or prioritized to establishments in the following cases:
Establishments that are not registered in the National Pharmacovigilance Network and that have been previously identified through the REPS and the Risk Map of the Directorate of Drugs and Biological Products.
Establishments from the National Pharmacovigilance Network that have not received a follow-up visit to their Pharmacovigilance programs in the immediate previous year.
Establishments that have been subject of complaints to Invima regarding aspects impacting primarily on the safety and efficacy of the products and/or processes for which they are in charge.
Establishments that are holders of health registrations, pharmaceutical laboratories/manufacturers, drug importers and establishments in general, that are related to alerts and/or sanitary measures, which manufacture and/or manage products considered as high-risk products (biological products, vaccines, narrow therapeutic margin, new products recently marketed in the country, among others).
Level of complexity:
For IPS, the highest complexity level institutions will be prioritized for considering that they have a greater impact on the community regarding coverage.
Establishments that do not have reports or in the last year have submitted zero harm reports for drug adverse events.
Establishments that in their last visit obtained a "not implemented” concept according to the assessment made by officials of the Pharmacovigilance Group, with the criteria set out in section 6 of this guideline.
Establishments that are prioritized by the Risk Unit of the Directorate of Drugs and Biological Products for being qualified under a high-risk category, according to the results of the Invima Risk Map evaluation.
Establishments in general that, due to knowledge of other ISC procedures, have failed to meet the current national regulations applicable to pharmacovigilance.

“It is hereby stated that the person(s) who hosted the visit refused to sign the minutes.

MAJOR (M):

MINOR (m):
Failure to comply with the item is not necessary to determine that the establishment has an implemented pharmacovigilance program.

Also, the way to communicate and minimize the risk associated with the use of drugs should be identified, through guides for safe use of drugs, therapeutic drug follow-up, interaction identification guidelines, among other strategies.

7.1.

b) Suspected drug/product and concomitant medication:
For each drug/product, state indication, start and end date of treatment, dosage, route, frequency of administration, moment os discontinuation, re-exposure or if the product is still used.
c) Adverse event information:
Start, description, severity and outcome of the event.
d) Reporter:
Name and address of the reporting institution, notifier’s name and/or identification, profession and contact number.
e) Analysis of adverse event causality according to the UPPSALA/WHO criteria.
Example of evidence:
On-line report, exclusive form or Invima’s current form (by physical or magnetic means or software) for the adverse event reports, last reports analyzed and notified by the institution’s pharmacovigilance program, last submission of reports to both Invima and the corresponding territorial health entity.
7.2.
The institutional reports of suspected drug adverse events are submitted to Invima, or any entity acting in that capacity, and to the territorial health entity according to the frequency established in the current regulation for serious and non-serious, expected and unexpected adverse events.

The institutional reports of suspected adverse events are submitted, according to the established frequency, to Instituto Nacional de Vigilancia de Medicamentos y Alimentos – Invima, or any entity acting in that capacity, and to the corresponding territorial health entity:

4.1.

Example of evidence:
Current procedure or other recording tool (by physical or magnetic means and/or software) used for the collection and evaluation of adverse events.
4.2.
The establishment has statistics on adverse events detected through their pharmacovigilance program.
MAJOR
The health registration holder, manufacturing laboratory and/or pharmaceutical industry establishment has documented, from their pharmacovigilance program, the generation of statistics and analysis from the collected data of adverse events.
This data will allow the decision-making and the generation of improvement plans.
Example of evidence:
Type of adverse events statistics used at the establishment, recent statistics reports of adverse events that have become known, meeting minutes of committees and/or presentations where these reports are shared with the actions taken in that regard, among other strategies.
4.3.
A systemic review is performed regarding the scientific literature and any other information on reported adverse events.
MINOR
The establishment has a systemic review of the scientific literature and any other bibliographic source for safety information on adverse events, which allows for analysis, decision-making and generation of improvement plans related to its products.
Example of evidence:
Documented reviews on the Invima’s website and other recognized bibliographical sources for adverse event information, articles on the products safety profile, warnings and sanitary measures, safety reports, notifications to healthcare professionals, patients and caregivers, notices by other reference health agencies, among others, that allow the evaluation of causality and analysis of adverse events detected by the establishment’s pharmacovigilance system.

PHARMACOVIGILANCE PROGRAM DOCUMENTATION

5.1.
The Pharmacovigilance program is documented at the establishment.
CRITICAL
Verify that the Health Registration Holders, Manufacturing Laboratories and other pharmaceutical establishments have a pharmacovigilance program with standards for detection, capture, analysis and information management mainly related to adverse events (adverse reactions, possible therapeutic failures, medication errors, off-label use).
Such a program should be a different document than other procedures and processes conducted at the establishment and it should contain a purpose, scope, responsible person(s), legal framework, definitions, standardized procedure for activities and responsible people for the detection, analysis, management , recording and processing of adverse events, clinical data analysis, reporting frequency and notification to the regulatory agency, processing of warnings and sanitary measures, training schedule for Pharmacovigilance topics, among others.
If necessary, it should contain annexes, flowcharts, guides, instructions and forms that complement the program and facilitate its implementation.
Example of the evidence:
Manual, procedures or document of the Pharmacovigilance Program standardized and updated for notification, record and processing of adverse events, warnings and health measures, analysis of information on products safety, submission of reports to the corresponding regulatory agency and other essential activities for the proper implementation of the program.
5.2.
Safety periodic reports and risk management plans (if applicable) are elaborated.
MAJOR
The establishment elaborates safety periodic reports and/or risk management plans – RMP for their products, which are available for consultation and for any regulatory agency if required.
Example of the evidence:
Safety periodic reports for the imported and marketed products, as well as for manufactured and exported products, which should be available at any time and easy to consult.
Risk Management Plans – RMP for products to be marketed.
Documented description of the frequency and relevant criteria for the elaboration of said reports.
5.3.
There is an updated record of post-marketing safety studies and risk/benefit analysis of products.

DRUG ADVERSE EVENT REPORTS

7.1.
For notification and submission of adverse event reports, a reference form containing at least the basic information of the current report form provided by Invima is adopted, or the reports are made through the on-line reporting platform for proper analysis.
CRITICAL
The establishment should have a reference form (CIOMS or FOREAM) for notification and report of adverse events, which should contain the basic information of the Invima’s current national report form, or do it through the on-line reporting platform completing the required information.
The report form adopted by the establishment should have the following information regarding:
a) Patient:
Identification, initials, gender and age, diagnosis, concomitant diseases, paraclinical and other relevant information.
b) Suspected drug/product and concomitant medication:
For each drug, state indication, start and end date of treatment, dosage, route, frequency of administration, moment its use was discontinued, re-exposure or if the product is continued to be used.
c) Adverse event information:
Start, brief description, severity and patient’s outcome of the event.
d) Reporter:
Name and address of the reporting establishment, notifier’s name and/or identification, profession and contact number.
e) Analysis of adverse event causality according to the UPPSALA/WHO criteria.
Example of evidence:
Report in reference form (by physical or magnetic means) or through the on-line platform for adverse events with the above described information.
7.2.
The reports of suspected adverse events are submitted to Instituto Nacional de Vigilancia de Medicamentos y Alimentos – Invima, or any entity acting in that capacity, according to the frequency established in the current regulation:
CRITICAL
Reports of suspected adverse events are submitted, according to the established frequency, to Instituto Nacional de Vigilancia de Medicamentos y Alimentos – Invima, or any entity acting in that capacity:
“During the last five working days every two months, a report containing non-serious, expected, and unexpected adverse events.
