시력을 담당하는 눈의 망막이 손상되는 당뇨망막병증은 당뇨병을 앓은 지 오래 돼야 생긴다고 생각한다. 그러나 사실과 다르다. 망막전문의들은 당뇨병 진단을 받은 환자 10명 중 1명은 진단 당시 당뇨망막병증이 있을 정도로 이미 눈에 합병증에 온 경우가 있다고 한다. 삼성서울병원 안과 강세웅 교수는 "당뇨망막병증은 당뇨병이 생기고 수년 뒤에 생기지만, 당뇨병 환자 대부분은 정확히 언제부터 병을 앓기 시작했는지 모르는 경우가 많다"며 "당뇨병을 진단 받은 시점에 망막검사를 꼭 한 번 받아야 한다"고 말했다.

◇실명 원인 1위… 인지도 떨어져

당뇨병으로 인해 생기는 합병증 중 가장 위험한 것이 바로 당뇨망막병증이다. 당뇨망막병증은 한 번 생기면 회복이 안 되며, 치료 시기를 놓치면 실명에까지 이를 수 있다. 대한안과학회 발표에 따르면 우리나라 성인의 실명 원인 1위는 당뇨망막병증이다. 병에 대한 인지도도 떨어져 있는 상황이다. 2012년 한국망막학회가 당뇨병 환자 260명을 대상으로 인식조사를 한 결과, 전체 응답자의 절반(54%)이 '당뇨망막병증에 대해 들어본 적이 없다'고 답했다. '들어본 적은 있지만 자세히는 모른다'는 답변이 31.2%, '당뇨병 진단 후에 망막검사를 한번도 받지 않았다'는 답변은 70.4%나 됐다.

◇혈당관리 잘 해도 위험

당뇨망막병증은 혈당 관리만 잘하면 문제 없을 것이라고 생각하는 사람이 많다. 그러나 당뇨망막병증은 높은 혈당보다 당뇨병을 오래 앓은 기간이 더 큰 위험인자이다. 혈당 조절을 잘 하더라도 당뇨병이 생긴 지 10년 후가 되면 환자의 상당수에서 당뇨망막병증이 생긴다. 세브란스병원 안과 고형준 교수는 "당뇨병을 앓은지 10년 후가 되면 50%에서 당뇨망막병증이 발생하고 30년 후에는 90%에서 망막병증이 발생한다는 보고가 있다"고 말했다.

강세웅 교수는 "혈당 외에 높은 혈압이나 콜레스테롤 등도 당뇨망막병증에 영향을 미친다"며 "다른 안과 질환이 있거나 유전적인 소인에 따라 당뇨망막병증이 잘 생기는 사람도 있다"고 말했다.

◇당뇨병 진단 후 바로 검사 해야

당뇨병으로 진단을 받으면 망막검사를 받아야 한다. 권형준 교수는 "눈동자를 키우는 산동(散瞳) 검사 후 망막을 보는 안저촬영을 해야 망막을 정확하게 살필 수 있다"고 말했다. 검사 후 이상이 없으면 1년에 한 번씩 정기검진을 받다가, 치료가 필요할 때 바로 치료를 시작해야 한다. 한길안과병원 망막센터 박영숙 센터장은 "치료를 빨리 시작해야 병이 악화되는 것을 막을 수 있다"고 말했다. 망막에 비정상적으로 혈관이 자란 경우는 레이저를 통해 혈관을 없애는 치료를 한다. 망막 중심부인 황반에 부종이 생기면 부종을 가라 앉히는 주사제를 쓴다.

당뇨망막병증은 초기에는 단순한 노안으로 오해하는 경우가 많다. 약간의 빛번짐이나 눈 속 출혈로 인한 비문증(눈 앞에 날파리가 날아다니는 듯한 증상) 등의 증상이 발생할 수 있지만 무심코 지나치기 쉽다. 박 센터장은 "증상에 의존하지 말고 정확한 검사를 받아야 한다"고 말했다.

☞ 망막

눈 내부에 있는 두께가 0.1~ 0.5㎜의 얇은 신경막. 망막은 눈에 들어온 빛 자극을 전기신호로 바꾸어 신경을 통해 대뇌에 전달해 사물을 인식할 수 있게 한다. 망막이 손상되면 시력저하, 실명까지 온다. 망막이 정상적인 기능을 하려면 망막에 분포하는 혈관이 건강해야 한다.

☞ 당뇨망막병증

당뇨병으로 인해 망막이 손상되는 병. 당뇨병으로 인해 혈당이 높아 망막 미세혈관의 혈액순환이 잘 안되면 망막은 혈액공급을 받기 위해 신생 혈관을 만든다. 신생 혈관은 쉽게 파열돼 출혈을 일으키고, 망막이 일그러지거나 부종이 생겨 나중에는 시력을 잃을 수도 있다.

Diabetic retinopathy, which damages the retinas responsible for sight, is thought to only occur after one has suffered from diabetes for a long time. However, the truth is different. Retinal specialists say that one in ten patients diagnosed with diabetes already has eye complications at the time of diagnosis, including diabetic retinopathy. “Diabetic retinopathy occurs many years after developing diabetes, but often the majority of diabetes patients do not know when their condition started,” said Professor Kang Se-woong, of Samsung Medical Center’s ophthalmology department. “It is vital to get a retina scan when they receive a diabetes diagnosis.”

◇ Leading cause of blindness, yet lack of awareness

Of the complications that develop from diabetes, diabetic retinopathy is the most dangerous. Once diabetic retinopathy develops, it cannot be cured. Blindness can even occur if the treatment period is missed. According to a statement from the Korean Ophthalmological Society, diabetic retinopathy is the leading cause of blindness in adults in Korea. Awareness of the disease is also low. In the results of a 2012 awareness survey of 260 diabetes patients conducted by the Korean Retina Society, half (54%) of all respondents answered that they had never heard of diabetic retinopathy. Thirty-one percent (31.2%) answered, “I’ve heard of it but don’t know the details,” and 70.4% answered, “I’ve never had a retinal screening since being diagnosed with diabetes.”

◇ Dangerous even with good blood sugar management

There are many people who think that as long as blood sugar is managed well, there won’t be any issues with diabetic retinopathy. However, having diabetes for a long time is more of a danger factor for diabetic retinopathy than high blood sugar. Even with good regulation of blood sugar, diabetic retinopathy occurs in a large number of patients 10 years after developing diabetes. “There is a study that shows diabetic retinopathy occurs in 50% of patients after 10 years of living with diabetes, and in 90% of patients after 30 years,” said Professor Go Hyeong-jun of Severance Hospital’s ophthalmology department.

“Besides blood sugar, things such as high blood pressure or cholesterol can have an effect on diabetic retinopathy,” said Kang. “There are also people in whom diabetic retinopathy easily develops, depending on if they have other ophthalmologic ailments or a genetic predisposition.”

◇ A screening is needed immediately after diabetes diagnosis

If you are diagnosed with diabetes, you must get a retina scan. “We can only precisely see the retinas if we take an ophthalmogram after doing a dilated eye exam that enlarges the pupils,“ said Professor Kwon Hyeong-jun. ((TR NOTE: Possibly a typo for Go Hyeong-jun)) If everything looks normal after the exam, you should have a regular check up once a year, and immediately begin treatment when treatment is needed. “You can only prevent the worsening of the disease if you begin treatment right away,” said Park Yeong-suk, head of Hangil Eye Hospital’s retina center. If blood vessels are growing abnormally in the retinas, treatment is performed with a laser to remove the blood vessels. If swelling or thickening occurs in the macula, which is near the center of the retina, an injection is used to soothe the macula. There are many instances in which diabetic retinopathy is mistaken for mere age-related farsightedness. Symptoms such as slight irradiation or floaters (a symptom that seems like a gnat is flying in front of the eye) from a hemorrhage in the eye can arise, but they are easy to ignore. “You need to get a precise exam rather than relying on symptoms,” said Park.


➢ Retina

Thin neural membrane in the inner part of the eye measuring 0.1-0.5mm thick. The retinas convert light stimulation entering the eye and transmit it to the brain through the nerves, making it possible to perceive objects. If the retinas are damaged, eyesight can deteriorate to the point of blindness. In order for the retinas to function normally, blood sugar distributed in the retinas must be at a healthy level.

➢ Diabetic retinopathy

Disease in which the retinas are damaged from diabetes. If blood sugar is high and blood circulation is poor in the microvessels of the retinas due to diabetes, the retinas will make new blood vessels in order to increase blood supply. New blood vessels can easily rupture and cause hemorraghing, and the retinas can be distorted or develop swelling, leading to later loss of vision.
